Basically, Squidward gets plastic surgery, drastically altering his features to an extent where Even the Guys Want Him. Attempting to escape from the constant swarm of stalkers, he then gets Spongebob to smash his face in the door a couple of times.
The results.... are the .GIF above. It made him even more stuffed up.
